Like I have said before, I used to do this more regularly, but it is time I think to give a shout out to the online casino brands that have been just plain great in all aspects. This time in all three's. First a little background to set the stage.

My main targeted area is the US so this is probably going to be the most useful to American affiliates, but some of these programs have options for the UK and AU as well, the data will probably only be mostly relevant to the United States specifically. In addition, I am a little slots and bonus heavy in my promotions making the conversion a bit less than other words like "review" or "gamble now". Now with all that said, it time to unveil...

The program that did the best:

Affactive - From making me happy with special deals for my players looking to make free money off of their no deposit bonuses, to going ahead and converting those same players into real money depositors. Sure that's the job of site your are making visable, but these guys are able to wrench it home, and get more people as regulars than anyone else I have been working with lately. It's what we all want, a base of regular depositing and happy gamblers and thats what I have been seeing. As an added bonus, they expand the demographics above to France as well. Oddly the rest are all marked predatory on the aff page but I am just being honest with who has been paying me and working well:

Ace Revenue - Even though I lost my manager to further her career in another field, Dan has stepped up and has been there when I need someone to talk to about stuff and yell at for making me miserable at times too. Fact is that Slot Madness and Silver Oak Casino have been blowing it up lately. Even with a small amount of overall traffic, they are obviously making US players happy as they not only grap new ones, but old ones keep putting their money in for more bonus gaming goodness. Payments take up till the middle to late next month to get here, still haven't seen anything for Nov yet for example, but they do always come eventually.

Lotus Affiliates - Ill keep this simple. Wicked fast payment with little to no fee's. Betonsoft slots and games from someone other than Genesys. Non-saturated like other older options. Likes to cross promote to their other brands and you keep the account. This I have seen with my own eyes. And to top it off, they are nice to me which actually means a lot in itself. I mean yes, integrity then money always comes first, but making me want to email my manager to see what's happening in the world of Lotus is also good. Heck, even their little blog is a decent read. Add in the possibility of promoting to an Asian market and you got a new team that has been a pleasure to work with and could be for you I am assuming.

For me, I would say:

Ace Revenue, Affactive and Affiliate Edge for the U.S
EuroPartners, Fortune Affiliates and Rewards Affiliates for the non-U.S

I can say for me, in this order over the past three months have been AffiliateEdge (however not so great this month...) AceRevenue and BettingPartners. I have been working all three for a long time now and all three have been great in terms of payments, conversions any answering any and every email I may send.

My first comment is about the OP's top 3...
My only issues with Affactive is their very player unfriendly cash-out policy which means new players (even if they just get lucky and have a big win, or (for example) if they deposit $1,000 and get a $3,000 bonus) will only get $500 every 2 weeks (at MOST of their casinos). This is bad for everyone involved IMHO. I have also heard that the guy who owns this group is "dodgy" at best... whether that is true or just a rumour I have no way of knowing for sure.
Ace Revenue are about as rogue as it gets - I wouldn't touch them with a 20ft barge pole!

Now, to my top 3 (by $) for the last 3 months...
32Red (MG)
Ladbrokes (MG)
400 Group (Rival)
I have to point out though, that all of those are only there because I got lucky and had a "high roller" at each during the last quarter - chances are my top 3 for the next 3 months will be 3 completely different groups!

I'm also a bit surprised to see the likes of AceRevenue and Europartners in these lists. I wouldn't trust either of them as far as I could throw them but to each his own, I guess.

As for AffActive, I do promote them. I do quite nicely with them too. Not 100% comfortable with their very low max withdrawal limits, though.

I'll give mine and then the AM's can have a good laugh that my top programs have such low balances ;) I'm revamping my sites after a long time that they were hacked, but still humorous... My top 3 are:
